Health Coaching Privacy Policy and Disclaimers

I, John Libonati, am committed to maintaining the privacy of my Clients and maintaining the security of any personal information received from them. You may be asked to provide basic personal information and complete forms. The information you provide will help me give you the best possible coaching. All your information will be kept securely and in complete confidence.

No Information provided by you will ever be made available for sale or use by a third party. The information used is solely for accurate assessment and coaching purposes. Your identity and circumstances will never be revealed to any third party.

Summary of Confidence Policy

I provide confidential coaching. This means your coaching will not be discussed with anyone. But there are some important legal and professional limits to confidentiality.

I am legally obliged to report the following to the appropriate authorities:

• Current child abuse including pedophilia.
• Threats of deliberate harm against the life of another person or oneself.
• Threats to commit terrorist acts.

Health Coaching does not diagnose illness, disease, or other physical or mental disorders. The Health Coach does not prescribe medical treatments or pharmaceuticals. The Health Coach is not a substitution for a medical examination or diagnosis and that it is recommended that you see a physician for any physical ailment that you might have.
