What Is Cheilosis?
[dropcap]C[/dropcap]heilosis is a painful feature of nutritional deficiencies, especially riboflavin and/or pyridoxine deficiency, and less frequently, from vitamin B12, folic acid, and iron. It is characterized by redness of the lips with cracking and weeping in the corner of the mouth.
Q: What is the consequence of weeping at the corners of the mouth?
A: The open sores that develop at the corners of the mouth invite infection by candida albicans, a yeast organism, and less commonly by bacteria normally present on the skin such as staphylococcus aureus.
These sores make opening the mouth very painful and so restrict eating, talking, socializing, and dental care.
What Is Cheilosis In Celiac Disease and/or Gluten Sensitivity?
- Cheilosis is a classic sign of multiple nutritional deficiencies caused by malabsorption in celiac disease due to the damaging effects of gluten on the small intestinal mucosa which must absorb the nutrients involved: riboflavin, pyridoxine, vitamin B12, folic acid, and iron.
- A recent Dutch study of 80 patients newly diagnosed with celiac disease found these deficiencies which may cause cheilosis: vitamin B6 (14.5%), folic acid (20%), vitamin B12 (19%), decreased iron storage (46%), and 32% had anemia.1
- It is well known that redness and soreness of the tongue with atrophied papillae is occassionally accompanied by angular stomatitis and less frequently by cheilosis and this constellation is linked to these deficiency states.2

Cheilosis is marked by these symptoms:
- Redness followed by
- Cracking.
- Weeping.
- Local soreness at the corners of the mouth with sharp, burning pain on opening the mouth.
How Does Cheilosis In Celiac Disease and/or Gluten Sensitivity Develop?
- Cheilosis results from B-complex deficiency, especially riboflavin and/or pyridoxine deficiency and less frequently, by vitamin B12, folate and iron deficiencies induced by celiac disease.3
Does Cheilosis Respond To Gluten-Free Diet?
Yes. Occurrence of celiac disease-related cheilosis resolves on a nutrient dense gluten free diet containing the missing nutrients.
6 Steps To Improve Cheilosis In Celiac Disease and/or Gluten Sensitivity:
- [dropcap]1[/dropcap]Remove the Trigger. Maintain a Strict, Nutritious Gluten Free Diet:

- A strict gluten free diet means removing 100% of wheat, barley, rye and oats from the diet.
- Cutting out bread and other obvious sources of gluten is not good enough for recovery. Even 1/8th teaspoon of flour or bread crumb is enough to sustain the inflammation that is damaging your small intestine, causing increased permeability (leaky gut) and allowing undigested gluten to enter your body where it can damage structures and function, and instigate immune inflammatory responses.
Correct Your Individual Nutritional Needs.

Recovery. You should begin to feel better within a week and notice more energy as inflammation subsides and the absorbing cells that make up the surface lining of your small intestine are better able to function.

Because gluten is inflammatory, eliminate OTHER inflammatory foods from your diet to reduce an additive effect to gluten. At the same time, try to eat foods that reduce inflammation (anti-inflammatory).

- Damaging Foods. In susceptible persons, includes corn, dairy (cow), and soy. Lactose, the sugar in any animal milk disrupts intestinal permeability causing leaky gut.5
- Allergenic Foods. Includes foods that trigger the immune sytem to produce IgE antibodies. Allergy testing is the usual way to discover these offending foods.
- Shelf Stable Processed Foods. Includes any that contain additives and preservatives. Look for them on the nutrition label of the box or package. Additives and preservatives also disrupt intestinal permeability causing leaky gut.5
- Fats. Limit deep fried foods, trans-fats, saturated fats (animal fat/butter), and EXCESSIVE omega-6 fatty acid oils like corn oil. Rancid fats, sodium caprate (a medium chain fat), and sucrose monester fatty acid (a food grade surfactant) induce significant disruption of the intestinal barrier that causes leaky gut.5.
- Excessive Refined White Flours (bran layer removed). Includes products made from them such as cookies, bread, cakes, pies. Bran contains the vitamins and minerals that metabolize grains and slows the otherwise rapid entry of sugar from their digestion into the bloodstream. Also disrupt intestinal permeability causing leaky gut.5
- Refined Sugars. Includes white sugar, corn fructose and high fructose corn syrup.
- Certain Spices. Includes paprika and cayenne pepper which disrupt intestinal permeability causing leaky gut.5

Certain prescription drugs cause deficiencies of riboflavin (vitamin B2), pyridoxine (vitamin B6), vitamin B12, folic acid, and iron that promote candidiasis.

The type and quantity of nutritional supplements that may be needed depend on which nutrients are deficient.
- Multivitamin/mineral combination once a day is useful to improve overall nutrient levels. This is a safe dose, but always check with your doctor to avoid interactions with medications.
- B-Complex supplying 100% to 300% of B vitamins. More than this is not absorbed and will deplete minerals from urine as the excess is excreted.
- Vitamin B12 as prescribed following blood test for status.
- Ferrous fumarate or gluconate as prescribed for low iron following blood test for status but do not take at same time as other mineral especially calcium because they compete for absorption.

- Eight glasses of water are recommended per day unless there is a contraindication such as kidney or heart disease. The Institute of Medicine recommends approximately 2.7 liters (91 ounces) of total water, from all beverages and foods, each day for women and 3.7 liters (125 ounces) daily of total water for men.
- If you are thirsty, drink water. Add fresh, squeezed lemon to water. Lemon is anti-inflammatory, alkalizing and provides vitamin C.
- Hydration Test: Urine should be pale yellow. Fingertips should be plump, without pruning but this may not be reliable when fingers are swollen with edema. Lips should be plump, without puckering. The feeling of thirst can be unreliable.
- What is wrong with soda, coffee, tea, and alcohol? These drinks are dehydrating, increase acid, and deplete nutrients.[/box]

They also tone the digestive muscles which improves peristalsis, thus aiding in the expulsion of gas from the stomach and intestine to relieve digestive colic and gastric discomfort.

- Grape is also bile stimulating and a cleansing remedy for sluggish digestion and laxative.
- Redbeets also stimulate and improve digestion and are easily digested.

- Lettuce also stimulates and improves digestion and is also an alterative, meaning it improves the function of organs involved with the digestion and excretion of waste products to bring about a gradual change.
- Potatoes are antispasmodic (due to atropine like properties) and a liver remedy.
Carminative Herb Remedies:
- Sage is also a digestive, astringent, bile stimulant and energy tonic that heals the mucosa. Drink as tea or use in cooking.
- Chamomile, lemon balm, and fennel, (as a tea) also help relieve nervous tension.
- Parsley also relieves indigestion.
- Rosemary as a tea and in cooking also is a nervous system tonic for stress and fatigue, bile stimulant, and can relieve headaches and indigestion.
- Thyme is also soothing remedy useful for stimulating digestion of rich, fatty foods.
Carminative Spice Remedies:
- Cloves are also antispasmodic.
- Nutmeg is also useful for indigestion.

What Do Medical Research Studies Tell About Cheilosis In Celiac Disease and/or Gluten Sensitivity?
RESEARCH STUDY SUMMARIES
“Vitamin and mineral deficiencies are highly prevalent in newly diagnosed celiac disease patients.” This study investigating the nutritional and vitamin/mineral status of current “early diagnosed” untreated adult celiac disease patients in the Netherlands found that vitamin/mineral deficiencies are still common in newly “early diagnosed” celiac disease patients including vitamin B6, vitamin B12, folate, and iron.
Eighty newly diagnosed adult celiac disease patients were included and a comparable sample of 24 healthy Dutch subjects was added to compare vitamin concentrations. Nutritional status and serum concentrations of folic acid, vitamin A, B6, B12, and (25-hydroxy) D, zinc, hemoglobin (Hb) and ferritin were determined (before prescribing gluten free diet). Almost all CD-patients (87%) had at least one value below the lower limit of reference. Specifically, for vitamin A, 7.5% of patients showed deficient levels, for vitamin B6 14.5%, folic acid 20%, and vitamin B12 19%. Likewise, zinc deficiency was observed in 67% of the CD-patients, 46% had decreased iron storage, and 32% had anemia. Vitamin deficiencies were barely seen in healthy controls, with the exception of vitamin B12. Vitamin/mineral deficiencies were counter-intuitively not associated with a (higher) grade of histological intestinal damage or (impaired) nutritional status. “Extensive nutritional assessments seem warranted to guide nutritional advices and follow-up in treatment of celiac disease.”6
