What Is Magnesium?

[dropcap]M[/dropcap]agnesium is an essential mineral predominantly found in the body within cells, where it is vital for their functions.

Here is a summary of what magnesium does in our body:

  1. Co-factor for over 300 enzymes involved in the metabolism of food components and synthesis of many compounds.
  2. Required for nerve transmission.
  3. Required for muscle activity (acts to relax muscles in opposition to calcium which acts to contract).
  4. Acts to maintain heart rhythm.
  5. Required for membrane transport and interactions.
  6. Required for glucose metabolism and energy production within cells.
  7. Component of bone mineralization and tooth formation.
  8. Plays a key role in calcium and phosphorus metabolism and management by parathyroid hormone.
  9. Maintains the structural and functional integrity of vital eye tissues such as lens.1

Only 1% of magnesium is found in blood, but the body must keep blood levels of magnesium constant.

What Is Crohn’s Disease?

[dropcap]C[/dropcap]rohn’s disease is an inflammatory bowel disease characterized by patchy inflamed areas involving the full thickness of the intestinal wall that can occur anywhere in the intestinal tract, in addition to, mucosal disease.

In Crohn’s disease there is ongoing immune activation which produces inflammation and ulceration but the cause is not known and the severity varies among patients. At diagnosis of Crohn’s disease, factors predictive of subsequent 5-year aggressive disease are an age below 40 years, the presence of perianal disease, and the initial requirement for steroids.1

Dysbiosis is a factor that develops in and worsens Crohn’s disease and stress is a factor in both of these conditions. Psychological stress activates multiple physiological processes aimed at maintaining balance within the body. These physiological processes also have the capacity to influence the composition of microbial communities in the digestive tract, and research now indicates that exposure to stressful stimuli leads to gut microbiota dysbiosis.2

While the relative abundance of many different bacterial types can be altered during stressor exposure, findings in nonhuman primates and laboratory rodents, as well as humans, indicate that bacteria in the genus Lactobacillus are consistently reduced in the gut during stress.2

Q: Is there a cure for Crohn’s disease?

A: Presently, Crohn’s disease cannot be cured. This condition has a course of remissions, when symptoms subside, and flares, when symtpoms get worse. Treatment is aimed to reduce flares and promote remission.

More than a feeling, this negative psychological status can range from mild to profound and can involve other parts of the body, causing physical problems such as poor digestion, constipation, weight gain or weight loss, mentrual irregularities in females, and impotence in males.

Q: How is depression identified?

A: According to the American Psychiatric Association, depression is a psychic condition that lasts for more than a month and involves four or more of these symptoms:

  • Abnormal appetite.
  • Diminished ability to concentrate or think properly.
  • Feelings of worthlessness.
  • Low energy or fatigue.
  • Physical inactivity or hyperactivity.
  • Sleep disturbances.
  • Thoughts of death.

Grief or sadness at the loss of a loved one or a similar event or remorse for sin is normal. However, depression that is prolonged usually involves imbalances of nerve chemicals called neurotransmitters. Imbalances can result from health disorders such as hormonal imbalances, low blood sugar, stress, drug side effects, or nutrient deficiencies.

[dropcap]S[/dropcap]troke in childhood is a medical emergency during which blood flow to an area of the brain is stopped, causing infarction or death of the area of cells in the brain fed by the blocked artery. Stroke is characterized by loss of muscle and brain function according to the location of the lost cells.

The brain is a highly active metabolic and complex organ of our body that performs important functions, therefore, any disruption in its normal functioning can have devastating effects on whole body.

After stroke, an immune response is initiated that leads to production of proinflammatory cytokines (chemicals) and gathering of various inflammatory cells like neutrophils, T-cells, macrophage, and monocytes to the affected area that exacerbate or worsen the ischemic (oxygen starved) injury.1

Arterial ischemic stroke is an important cause of acquired brain injury in children.2 This stroke results from loss of adequate blood flow through an artery that supplies the affected area of the brain with oxygen and nutrients.

A recent study found the majority of  children (85%) had focal features (most commonly one sided weakness or paralysis) at presentation to their medical practitioner. Seizures were more common in younger children a year old or younger and headache was more common in children 5 years or older.3

Who is at Risk in the General Population?

  • A British study in 2014 found the crude incidence of childhood arterial ischemic stroke was 1.60 per 100,000 per year.
  • The incidence of arterial ischemic stroke was highest in children aged under 1 year (4.14 per 100,000 per year).
  • There was no difference in the risk of arterial ischemic stroke between sexes.
  • Asian and black children were at higher risk than were white children.3

What Is Iron Deficiency Anemia?

[dropcap]I[/dropcap]ron deficiency anemia is a blood cell disorder that is characterized by formation of small, pale red blood cells, causing tissue hypoxia. Hypoxia is the inability to meet the demands of the body for oxygen.

Q: Why do small, pale red blood cells cause tissue hypoxia?

A: Small, pale red blood cells (erythrocytes) cause tissue hypoxia because they are not able, as do normal erythrocytes, to pick up adequate oxygen from the lungs and carry it to cells that use oxygen.

Red blood cell production and function are dependent on a sufficient level of iron in the body and also the ability to use available iron to make hemoglobin in red blood cells.

Hemoglobin is a protein that binds oxygen in red blood cells to be carried by the bloodstream to cells throughout the body. In iron deficiency anemia,  hemoglobin in females is below 12.5g/dl (normal range is 12.5 to 16g/dl) and in males it is below 13.5g/dl (normal range is 13.5 to 17.5g/dl).

Iron must be obtained from the diet, since the body cannot make it, but there are various factors that can interfere with absorption and use in the body, causing anemia. Iron absorption from the gut first requires ionization, or gaining a positive electrical charge, in the strongly acidic environment of stomach juice. Ionized iron, only, can be absorbed in the duodenum, which receives the acidic contents of the stomach before it is neutralized further along.

Dietary iron can be heme or non-heme depending on the food source. Heme iron obtained only from animal food sources is absorbed into the bloodstream by active transport across the brush border (microvilli) which cover the multitudinous villi of the small intestinal lining.

Non-heme iron obtained from plants must bind with apoprotein after entering the enterocyte (surface cell of small intestinal lining) to be ferried to the underlying basolateral membrane and exited by active transport into the bloodstream.

Frequently, chronic anemia due to iron deficiency is accompanied by increased platelets, and this thrombocytosis resolves with iron repletion (normal iron level). Conversely, in severe iron deficiency anemia, patients may have thrombocytopenia (low platelets), which also resolves with iron therapy.1

Almost two-thirds of iron in the body is found in hemoglobin, the protein in red blood cells that carries oxygen to tissues. Smaller amounts of iron are found in myoglobin, a protein that helps supply oxygen to muscle, and in enzymes that assist biochemical reactions.

Iron is also found in proteins that store iron for future needs and that transport iron in blood.

Q: How are iron stores regulated?

A:Iron stores are regulated by intestinal iron absorption.1That is, whenever iron is needed, more iron is absorbed than when iron is sufficient. This mechanism prevents excess iron in the body which is harmful.

In the brain, iron is necessary to ensure oxygenation and to produce energy in the cerebral parenchyma (via cytochrome oxid. ase), and for the synthesis of neurotransmitters and myelin.

Iron concentrations in the umbilical artery are critical during the development of the fetus, and in relation with the IQ in the child.2

What Is Vitamin B3 (Niacin)?

[dropcap]N[/dropcap]iacin is an essential water-soluble B vitamin that is required by all cells of the body.

During digestion of food containing it, niacin (the form in food) is changed in the small intestines to the active form niacinamide (niacin plus an amide group), which is then absorbed into the bloodstream. 

Niacinamide is converted by the body into co-enzymes which are present in all cells. These are niacinimide adenine dinucleotide (NAD) and NADP. NADP is formed when the body adds a phosphate to NAD.

Q: How do these enzymes work?

A: These enzymes function in oxidation-reduction reactions essential for release of energy from carbohydrates, fats, and proteins and are needed as components for more than 200 enzymes involved in metabolism.

In addition to producing energy, niacinamide is essential for healthy skin and the mucosal lining of the digestive tract, normal functioning of the brain and nervous system, and production of steroid hormones in adrenal glands and hormones in sex glands.  Functions are more fully described below.

Urinary excretion of niacin cannot be detected when vitamin intake is below the required levels. On the other hand, when intake exceeds saturation in the body, the vitamin and/or its metabolites are actively excreted into urine to prevent excessive toxicity of the vitamins.1
