H ypoprothrombinemia is a deficiency of prothrombin (clotting factor II) in the blood that is characterized by impaired hemostasis in response to trauma or a laceration.

Q: What is hemostasis and how is it altered by a deficiency of prothrombin?

A: Hemostasis encompasses the tightly regulated processes of blood clotting, platelet activation, and blood vessel repair.1

Prothrombin is a protein clotting factor present in blood that is involved in the first part of hemostasis, which is blood clotting or coagulation. Vitamin K is required for prothrombin production.

When a laceration or wound is sustained, prothrombin is converted to the enzyme thrombin. Thrombin in turn acts on fibrinogen to convert it to fibrin which then forms the framework of a clot to stop bleeding. Deficiency of prothrombin prevents this series of events and bleeding is not properly stopped.

After the clotting process of hemostasis would come the second part, platelet activation. Eventually, coagulation and platelet activation are switched off by blood-borne inhibitors.

How Gluten Causes Alopecia Areata

Alopecia-Areata-wikimedia-227x300[3]Do you recognize this circular hair loss pattern? It is called alopecia areata. According to the National Alopecia Areata Foundation, “no FDA approved treatment or cure exists yet.”

No cure, eh? It is amazing how many times I hear this “no cure” declaration when I KNOW people who have been cured of the condition in question, in this case, alopecia areata. Maybe they mean no DRUG exists that cures it.

I was recently shopping at a department store to pick up some new exercise shorts when the security guard walked past me.

He was a young man, about twenty-five, medium height with a thick build, pale skin and short cropped black hair. I noticed several round spots where hair was missing from the side and back of his head. It looked like he had taken an electric razor and buzzed those places bare.

I said hi. He replied with a smile and a hello. I told him I didn’t mean to pry, but I noticed the bald spots and asked whether he got them in an accident. He said no, they just started appearing a few years ago and he’s tried everything to get the hair to grow back.

I asked if he was taking zinc supplements. He said no and asked why…

According to the Health Guide, alopecia areata is caused by an immune reaction and targets the area and zinc deficiency. The immune reaction can be caused by gluten and the zind deficiency by either not eating enough, or more likely, by malabsorption of zinc – a common problem in people with celiac disease who are not on a gluten-free diet. Taking zinc and following a gluten-free diet can turn around alopecia areata so that the hair grows back.

The guard thanked me about ten times. He was ecstatic to discover that he might have hope of getting his hair back after all.

Now, I don’t know how smart it is to walk up to a stranger and start advising them on their hair loss, but in this case I already knew a number of people whose alopecia areata resolved after they went gluten free.

N eutropenia  is a blood disorder characterized by presence of an abnormally low number of neutrophils.

Neutrophils are white blood cells (leukocytes) that serves as the primary defense against infections by destroying bacteria in the blood. 

Specfically, neutrophils are a type of granulocyte that contain granules filled with potent chemicals to break down the microbes they ingest. Some of these chemicals, such as histamine, also contribute to inflammation and allergy.

The process of eating and digesting microbes is called phagocytosis. Neutrophils are phagocytes.1

Q: How do neutrophils eat microbes?

A: Segmented neutrophils  are the mature phagocytes that migrate through tissues to destroy microbes and respond to inflammatory stimuli. Segmented neutrophils comprise 40-75 % of the peripheral leukocytes. They are usually 9 to 16 µm in diameter. The nuclear lobes, normally numbering from 2 to 5, may be spread out so that the connecting filaments are clearly visible, or the lobes may overlap or twist. The chromatin pattern is coarse and clumped. The cytoplasm is abundant with a few nonspecific granules and a full complement of rose-violet specific granules.1

What Is Transient Erythroblastopenia?

T ransient erythroblastopenia is a rare disorder of red blood cell formation characterized by brief, reversible disappearance of erythroblasts (red blood cell precursors) in the bone marrow of children.

Q: What do the red blood cells look like?

A: Circulating red blood cells appear normal so that the anemia which develops is a normocytic (normal cellular) anemia in contrast to microcytic (small cell) anemia in iron deficiency or macrocytic (large cell) anemia in vitamin B12 or folic acid deficiency.

Because new red blood cells are failing to mature, packed red blood cell transfusions may be necessary in severe anemia.

In adults, this disorder is called pure red blood cell anemia.

 What Is Geographic Tongue?

G eographic tongue is a painful alteration in tongue tissue that is characteristic of riboflavin (vitamin B2) deficiency.1

Q: How does the tongue surface appear?

A: Areas of inflamed tissue with flattened papillae develop among areas of normal tissue, giving a map-like appearance to the tongue surface. There are often swollen papillae that hurt or sting, such as is seen in the photo to the left.

A diet that does not provide adequate amounts of riboflavin and any condition that 1) interferes with riboflavin absorption from the gut, 2) depletes the vitamin from the body, 3) overuses the vitamin in the body, or 4) adversely affects its use in the body will result in riboflavin deficiency causing geographic tongue.

Riboflavin is essential for converting foodstuffs into energy, maintaining the normal lifespan of red blood cells and maintaining healthy skin, eyes, hair, and nervous system. Riboflavin is converted into its active forms, flavin adenine dinucleotide (FAD) and flavin mononucleotide (FMN). These enzymes are primarily involved as co-factors in oxidation reduction reactions that are essential for cellular energy production and respiration.

Riboflavin is required for the conversion of pyridoxine (vitamin B6) to its functional form and the production of niacin (vitamin B3) from the amino acid tryptophan. Riboflavin has a role in antioxidant status by activating glutathione reductase, which regenerates used glutathione, a powerful antioxidant.

Respiratory infection, exercise, certain diseases, drugs and hormones can adversely influence riboflavin status.2 Stress is a major depleter of riboflavin.

Understanding Riboflavin (Vitamin B2) Deficiency in Celiac Disease

Riboflavin is a micronutrient, also known as Vitamin B2, which performs many important functions in the body. Riboflavin is a water-soluble vitamin that releases energy from carbohydrates, amino acids, and lipids. It plays a key role in specific amino acid production and provides antioxidant protection. Riboflavin is essential for growth and production of red blood cells and maintaining healthy skin, eyes, hair, and nervous system.

Riboflavin depletion and/or deficiency is common before starting the gluten-free diet treatment.
