Lactose IntoleranceWhat Is Lactose Intolerance?

[dropcap]L actose intolerance is a well known symptom of carbohydrate malabsorption characterized by inability to properly digest lactose, the sugar in milk, due to low lactase digesting activity in the small intestine.

Lactase is an enzyme produced by specialized cells in the tips of villi within the brush border of the small intestinal lining that digests lactose.

Lactose is a disaccharide, or double sugar, made up of a molecule of glucose and a molecule of galactose (simple sugars). Lactose cannot be absorbed as is into the body unless it is broken down by lactase into glucose and galactose.

Q: How does lactose cause diarrhea?

A: If lactose is not broken down, undigested lactose moves into the colon where it acts osmotically to draw water from the walls of the colon into the lumen, which causes diarrhea by the unnatural amount of water. How much water? Can be 2 liters! Meanwhile, the normal population of colonic bacteria ferment the undigested lactose, generating short-chain fatty acids and hydrogen gas, resulting in bloating pain and flatulence.

Testing for intolerance is based on the action of lactose to increase intestinal permeability.1 Positive response to a breath hydrogen test (BHT), involving 1 – 3 hours of time post ingestion of lactose test dose, signifies malabsorption in the small intestine and fermentation in the colon. If BHT is positive before 60 minutes, the result implies bacteria is abnormally present in the small intestine, causing fermentation there.

Endoscopy is used to measure activity of lactase in a tissue sample.

Treatment is avoidance of milk and dairy products that contain lactose. Nevertheless, milk treated with lactase enzyme can be safely consumed. Fermented milk products like yogurt or kefir are safe because the lactose has been digested by organisms. If there is also a problem with milk protein, then no milk is acceptable that comes from the cow or animal that is causing the allergy.

What Is Lactose Intolerance In Celiac Disease and/or Gluten Sensitivity?

  • Lactose intolerance is a classic symptom in patients with celiac disease showing dysfunction of the absorptive mucosal lining damaged by gluten.
  • Lactose intolerance is also a feature of collagenous colitis which is a complication of celiac disease.2

How Prevalent Is Lactose Intolerance In Celiac Disease and/or Gluten Sensitivity?

Lactose intolerance affects about 50% of people with untreated celiac disease.3

What Are The Symptoms Of Lactose Intolerance?

Lactose intolerance is marked by these symptoms:

  • Diarrhea that can produce hard to control stooling.
  • Bloating with or without distention.
  • Cramps.
  • Flatulence with smelly farts.
  • Abdominal pain.
  • Some persons experience faintness, coldness, and a cold sweat.

How Does Lactose Intolerance Develop In Celiac Disease and/or Gluten Sensitivity?

  • Lactose intolerance results secondarily in celiac disease from lactase deficiency due to diffuse villous atrophy (damage) resulting from inflammation that destroys the enzyme producing cells.
  • Failure of digestion in the small intestine leaves lactose free to travel to the colon where its presence cause the symptoms.

Does Lactose Intolerance Respond To Gluten-Free Diet?

Yes. Celiac disease-related lactose intolerance should improve in 90% of newly diagnosed celiacs.4

6 Steps To Improve Lactose Intolerance In Celiac Disease and/or Gluten Sensitivity:

  • [dropcap]1 Remove the Trigger. Maintain a Strict, Nutritious Gluten Free Diet:

  • Fats. Limit deep fried foods, trans-fats, saturated fats (animal fat/butter), and EXCESSIVE omega-6 fatty acid oils like corn oil.   • Eight glasses of water are recommended per day unless there is a contraindication such as kidney or heart disease.   • If you are thirsty, drink water. Add fresh, squeezed lemon to water. Lemon is anti-inflammatory, alkalizing and provides vitamin C.
  • Hydration Test: Urine should be pale yellow. Fingertips should be plump, without pruning but this may not be reliable when fingers are swollen with edema. Lips should be plump, without puckering. The feeling of thirst can be unreliable.

What Do Medical Research Studies Tell About Lactose Intolerance In Celiac Disease and/or Gluten Sensitivity?

RESEARCH STUDY SUMMARIES

Unclear abdominal discomfort: pivotal role of carbohydrate malabsorption.” This study investigating the prevalence of symptomatic lactose and fructose malabsorption in 2,390 patients with IBS-like symptoms based on Rome II criteria found that carbohydrate malabsorption is a frequent but underestimated condition in patients with IBS-like symptoms although diagnosis can be easily confirmed by H2 breath testing. Celiac disease was found in 1 of 161 patients by upper endoscopy examination.

Patients with unclear abdominal discomfort underwent lactose (50 g) and fructose (50 g) hydrogen (H2) breath tests and depending on the results further testing with 25 g fructose or 50 g glucose, or upper endoscopy with duodenal biopsies. Additionally, this population was investigated regarding the prevalence of small intestinal bacterial overgrowth (SIBO) based on glucose breath test and celiac disease.

Of the 2,390 patients with IBS-like symptoms, 848 (35%) were symptomatic lactose malabsorbers and 1,531 (64%) symptomatic fructose malabsorbers. A combined symptomatic carbohydrate malabsorption was found in 587 (25%) patients. Severe fructose malabsorbers (pathologic 25 g fructose test) exhaled significantly higher H2 concentrations in the 50 g test than patients with negative 25 g fructose test (P < 0.001).7

“Celiac sprue among US military veterans: associated disorders and clinical manifestations.” This study investigating the clinical manifestations of celiac disease related to malnutrition and analyzing the associations between celiac disease and other diagnoses confirmed the known association of celiac disease with lactase deficiency.8
