Ferritin is a\u00a0protein within cells that stores iron and releases it as needed.<\/p>\n<p style=\"text-align: justify\">According to the World Health Organization (WHO) mild anemia corresponds to a hemoglobin (Hb) of \u00a09.5 g\/dL, moderate anemia to a Hb of \u00a08 g\/dL but less than 9.5 g\/dL, and severe anemia to a Hb of less than 8.0 g\/dL.<\/p>\n<h2>What Is Refractory Iron Deficiency Anemia In Childhood In Celiac Disease and\/or Gluten Sensitivity?<\/h2>\n<p><!--more--><\/p>\n<ul class=\"cp_check red\">\n<li style=\"text-align: justify\">Refractory iron deficiency anemia\u00a0in childhood is a classic symptom of celiac disease and is a common presenting feature of untreated celiac disease.<\/li>\n<li style=\"text-align: justify\">A pediatric study evaluating the effect of iron supplementation, in addition to a gluten free diet, on the blood profile of children with celiac disease demonstrated that iron deficiency anemia is commonly associated with celiac disease and the iron deficiency state continues a long time even after excluding gluten from the diet and iron supplementation.<a href=\"#footnote_1_898\" id=\"identifier_1_898\" class=\"footnote-link footnote-identifier-link\" title=\"Karnam US, Felder LR, Raskin JB. Prevalence of occult celiac disease in patients with iron-deficiency anemia: a prospective study. Southern Medical Journal. Jan 2004;97(1):30-4.\">1<\/a><\/li>\n<li style=\"text-align: justify\">Researchers found that an abnormal level of ferritin (iron) present in blood indicating iron deficiency\u00a0does not correspond to the degree of damage present in the small intestinal lining (villous atrophy) caused by celiac disease. That is, low iron stores can develop in children with any degree of villous atrophy, from minimal to extensive.<\/li>\n<li style=\"text-align: justify\">Furthermore, in follow-up evaluation of iron deficient children on a gluten free diet, the average hemoglobin levels were comparable with healthy children (controls), but they (subjects) continued to have abnormally small blood cells, lower blood iron levels and higher than normal total iron binding\u00a0capacity (TIBC) results. TIBC is a blood test to see if there is too much or too little iron in the blood. Iron moves through the blood attached to a protein called\u00a0transferrin. This test indicates how well that protein can carry\u00a0iron in the blood.<a href=\"#footnote_2_898\" id=\"identifier_2_898\" class=\"footnote-link footnote-identifier-link\" title=\"http:\/\/www.nlm.nih.gov\/medlineplus\/ency\/article\/003489.htm\">2<\/a><\/li>\n<li style=\"text-align: justify\">Celiac disease\u00a0patients with\u00a0anemia\u00a0had more severe\u00a0disease\u00a0than those without\u00a0anemia. When compared with celiac disease\u00a0patients without\u00a0anemia, those with\u00a0anemia\u00a0had significantly longer duration of symptoms, lower albumin levels, and higher anti-tissue transglutaminase fold rise, and a higher proportion had abnormal d-xylose tests and severe villous abnormalities.<a href=\"#footnote_3_898\" id=\"identifier_3_898\" class=\"footnote-link footnote-identifier-link\" title=\"Singh P, Arora S, Makharia GK. Presence of anemia in patients with celiac disease suggests more severe disease. Indian J Gastroenterol. 2013 Nov 19.\">3<\/a><\/li>\n<li style=\"text-align: justify\">A questionnaire study investigating screening practices for celiac disease in patients with iron-deficiency anemia (IDA) because no screening guidelines exist in the literature found that practicing hematologists infrequently screen for celiac disease in IDA. Only 8.6% believed all patients with IDA should be screened for celiac disease. Physicians who have recently finished their fellowship, those who see a high volume of patients with IDA, and\u00a0those who saw predominately pediatric patients with IDA are more likely to screen for celiac disease.<a href=\"#footnote_4_898\" id=\"identifier_4_898\" class=\"footnote-link footnote-identifier-link\" title=\"Smukalla S, Lebwohl B, Mears JG, Leslie LA, Green PH. Epub 2012 Aug 19.\">7<\/a><\/li>\n<\/ul>\n<h2>What Are The Symptoms Of Iron Deficiency Anemia In Childhood?<\/h2>\n<p style=\"text-align: justify\">Refractory iron deficiency anemia\u00a0in childhood is marked by these features:<\/p>\n<ul class=\"cp_bullet red\">\n<li><span style=\"text-align: justify\">Pallor or \u00a0paleness of skin and mucous membranes.<\/span><\/li>\n<li><span style=\"text-align: justify\">Fatigue.<\/span><\/li>\n<li>Drowsiness.<\/li>\n<li>Weakness.<\/li>\n<li>Irritability.<\/li>\n<li>Loss of appetite.<\/li>\n<li>Anxiety.<\/li>\n<li><span style=\"text-align: justify\">Dyspnea (shortness of breath) on exertion, such as running.<\/span><\/li>\n<li><span style=\"text-align: justify\">Mental sluggishness or apathy.<\/span><\/li>\n<li><span style=\"text-align: justify\">Reduced memory\/learning.<\/span><\/li>\n<li><span style=\"text-align: justify\">Negativity.<\/span><\/li>\n<li>Inability to pay attention.<\/li>\n<li>Headache.<\/li>\n<li>Visual impairment (blurry).<\/li>\n<li>Alopecia (loss of hair).<\/li>\n<li>Dry and dull hair.<\/li>\n<li><span style=\"text-align: justify\">Inc<\/span><span style=\"line-height: 1.5em\">reased susceptib<\/span><span style=\"line-height: 1.5em;text-align: justify\">ility to infection.<\/span><\/li>\n<li>Systolic heart murmur may develop.<\/li>\n<\/ul>\n<h2>How Does Refractory Iron Deficiency Anemia Develop In Celiac Disease and\/or Gluten Sensitivity?<\/h2>\n<ul class=\"cp_check red\">\n<li style=\"text-align: justify\">Refractory iron deficiency anemia\u00a0in childhood results from malabsorption of iron from the small intestine in celiac disease.<\/li>\n<li style=\"text-align: justify\">Study of ferrokinetics has shown that an anemia was caused by deficiency in many elements with preponderence of iron-deficiency erythropoiesis (blood cell production).<\/li>\n<li style=\"text-align: justify\">The most sensitive and reliable indicators of early detection of latent anemia without decrease in hemoglobin level were ferritin content, the coefficient of saturation of transferrin and transferrin&#8217;s receptors.<a href=\"#footnote_8_898\" id=\"identifier_8_898\" class=\"footnote-link footnote-identifier-link\" title=\"Umarnazarova ZE. Oct-Nov 2003;(7):63-7.\">8<\/a><\/li>\n<\/ul>\n<h2>Does Refractory Iron Deficiency Anemia Respond To Gluten-Free Diet?<\/h2>\n<p style=\"text-align: justify\">Yes. Iron deficiency anemia in celiac disease responds slowly to gluten free diet containing iron and folic acid supplementation.<a href=\"#footnote_9_898\" id=\"identifier_9_898\" class=\"footnote-link footnote-identifier-link\" title=\"Karnam US, Felder LR, Raskin JB. Prevalence of occult celiac disease in patients with iron-deficiency anemia: a prospective study. Southern Medical Journal. Jan 2004;97(1):30-4.\">9<\/a><\/p>\n<ul class=\"cp_check green\">\n<li><em><span class=\"dropcap\"><strong>1<\/strong><\/span><strong>Remove the Trigger. Maintain a Strict, Nutritious Gluten Free Diet:<\/strong><\/em><\/li>\n<\/ul>\n<p style=\"text-align: justify\"><div class=\"box shadow\"><div class=\"box-inner-block\"><i class=\"tieicon-boxicon\"><\/i>\n\t\t\t<b>Treatment<\/b>. This condition responds to the complete elimination of gluten, which is the required treatment that improves both anemia and gut health.<\/p>\n<ul class=\"cp_bullet green\">\n<li>Gut health is the foundation to restore ALL health.<\/li>\n<li style=\"text-align: justify\">A strict gluten free diet means removing 100% of wheat, barley, rye and oats from the diet.<\/li>\n<li style=\"text-align: justify\">Cutting out bread and other obvious sources of gluten is not good enough for recovery. Physicians who have recently finished their fellowship and those who see a high volume of patients with IDA are more likely to screen for celiac disease.<\/p>\n<p>A survey was e-mailed to members of the American Society of Hematology to survey hematologists to determine rates of celiac disease screening.<br \/>\nThere were 385 complete responses from 4551 e-mails. Most respondents were practicing clinicians (74%), clinical researchers (10%), or laboratory researchers (6%). Specialists in benign hematology accounted for 45% of respondents, oncologists accounted for 33%, and specialists in malignant hematology accounted for 22%. Sixty-six (60.6%) were classical type, 41 (37.6%) were atypical type and 2 (1.8%) were silent type.
