Courtesy almostadoctor.com\" width=\"239\" height=\"300\" srcset=\"https:\/\/glutenfreeworks.com\/health\/wp-content\/uploads\/sites\/10\/liver-lobule-239x300.png 239w, https:\/\/glutenfreeworks.com\/health\/wp-content\/uploads\/sites\/10\/liver-lobule.png 400w\" sizes=\"auto, (max-width: 239px) 100vw, 239px\" \/><\/a><p id=\"caption-attachment-8622\" class=\"wp-caption-text\">Figure 1: Normal Liver Lobule. <em>Courtesy almostadoctor.com<\/em><\/p><\/div>\n<h2>What Is Autoimmune Cholangitis?<\/h2>\n<p style=\"text-align: justify\"><span class=\"dropcap\">A<\/span>utoimmune cholangitis is a rare inflammatory self-targeted disease of the biliary tract produced by high levels of immunoglobulin G (IgG) antibodies and antinuclear antibodies (ANA).<\/p>\n<p style=\"text-align: justify\">Autoimmune cholangitis\u00a0is characterized by progressive inflammation and scarring that leads to narrowing and destruction of the bile ducts and development of biliary cirrhosis of the liver.<\/p>\n<p style=\"text-align: justify\">Autoimmune cholangitis is also termed\u00a0anti-mitochondrial antibody-negative primary biliary cirrhosis because there is no production of anti-mitochondrial antibodies.<\/p>\n<p style=\"text-align: justify\"><span style=\"color: #000000\"><strong>Q:<\/strong><\/span><span style=\"line-height: 1.5em\"> What happens when bile ducts can no longer transport bile out of the liver?<\/span><\/p>\n<p style=\"text-align: justify\"><span style=\"color: #000000\"><strong>A:<\/strong><\/span>\u00a0<span style=\"line-height: 1.5em\">Bile that cannot be removed from the liver by the biliary tract backs up and damages the liver, causing cirrhosis.\u00a0<\/span><span style=\"line-height: 1.5em\">In Figure 1, bile back up would directly damage the hepatic (liver) cells seen here as grey &#8220;blocks.&#8221;\u00a0<\/span><\/p>\n<p style=\"text-align: justify\"><span style=\"line-height: 1.5em\">Bile is\u00a0<\/span><span style=\"line-height: 1.5em\">a greenish brown liquid\u00a0<\/span><span style=\"line-height: 1.5em\">continually made by the liver cells (hepatocytes) from phospholipids salt, cholesterol, and aging blood cells that it removes from circulation.\u00a0<\/span><\/p>\n<p style=\"text-align: justify\"><span style=\"line-height: 1.5em\">Bile also carries away waste products produced by normal metabolism and\u00a0toxic substances that are removed by the liver\u00a0<\/span><span style=\"line-height: 1.5em\">for eventual elimination in stool.<\/span><span style=\"line-height: 1.5em\">\u00a0<\/span><span style=\"line-height: 1.5em\">As such, bile\u00a0must continually flow out of the liver to prevent build-up in the liver.\u00a0<\/span><\/p>\n<p style=\"text-align: justify\"><span style=\"line-height: 1.5em\">Bile from the hepatic cells drains into small tubes called canniculi and then into larger bile ducts\u00a0<\/span><span style=\"line-height: 1.5em\">to be carried out of the liver\u00a0<\/span><span style=\"line-height: 1.5em\">to the gall bladder for storage until needed to aid in the digestion and absorption of fat from the small intestine.\u00a0<\/span><span style=\"line-height: 1.5em\">\u00a0 Bile emulsifies fat eaten in the diet so that the pancreatic enzyme called lypase can break it down into its fatty acid and glycerol components.\u00a0<\/span><\/p>\n<p style=\"text-align: justify\">Unfortunately, autoimmune cholangitis is difficult to diagnose because it mimics many other diseases such as primary sclerosing cholangitis, sarcoidosis, cancer and lymphoma.<\/p>\n<h2>What Is Autoimmune Cholangitis In Celiac Disease and\/or Gluten Sensitivity?<\/h2>\n<p><!--more--><\/p>\n<ul class=\"cp_check red\">\n<li style=\"text-align: justify\">Autoimmune cholangitis is an associated liver disorder in celiac disease.<\/li>\n<li style=\"text-align: justify\">The association between celiac disease and other immune disorders may be due to the sharing of a common genetic background, such as HLA antigens.\u00a0However, in a very large study, involving 909 patients with celiac disease, it was found that the development of immune disorders in celiac disease was clearly related to the duration of exposure to gluten.<a href=\"#footnote_1_312\" id=\"identifier_1_312\" class=\"footnote-link footnote-identifier-link\" title=\"La Villa G, Pantaleo P, Tarquini R, Cirami L, Perfetto F, Mancuso F, Laffi G. Dec 2002;97(12):3196-8.\">2<\/a><\/p>\n<h2>What Are The Symptoms Of Autoimmune Cholangitis?<\/h2>\n<p>Autoimmune cholangitis is marked by these symptoms:<\/p>\n<ul class=\"cp_bullet red\">\n<li>Progressive weakness.<\/li>\n<li>Pruritis (itching).<\/li>\n<li><span style=\"line-height: 1.5em\">Chronic elevated serum liver enzymes.<\/span><\/li>\n<li>Development of yellow skin and whites of eyes due to back-up of bile in the liver.<\/li>\n<\/ul>\n<h2>How Does Autoimmune Cholangitis In Celiac Disease and\/or Gluten Sensitivity Develop?<\/h2>\n<ul class=\"cp_check red\">\n<li>Autoimmune cholangitis results from autoimmune mechanism.<\/li>\n<\/ul>\n<h2>Does Autoimmune Cholangitis Respond To Gluten-Free Diet?<\/h2>\n<p style=\"text-align: justify\">Yes. Resolution of celiac disease-related pruritis (itching) and elevated liver biochemistries occurred on a gluten free diet, avoiding the need for immunosuppressive therapy.<a href=\"#footnote_2_312\" id=\"identifier_3_312\" class=\"footnote-link footnote-identifier-link\" title=\"Sedlack RE, Smyrk TC, Czaja AJ, Talwalkar JA.
